My team is always firefighting. Is that normal?
It is common and it is expensive. Firefighting means the process only works when someone competent intervenes, so the business is buying the same fix over and over in the form of overtime and stress.
Decisions get made but nothing happens after. What fixes that?
Ownership and cadence, in that order. Every decision needs one name against it, a date, and one place where its status is visible. Decisions assigned to a team are decisions assigned to nobody.
Can you take over just one workflow?
Yes, and it is often the best way to start. One workflow, mapped and rebuilt properly, gives you a working example and a real measurement rather than a plan.
What is the difference between fractional operations and a virtual assistant?
A virtual assistant executes tasks you define. Fractional operations decides what the tasks should be, redesigns the process behind them, and puts the system in place so the work stops depending on any one person.
Who does the work?

What if our processes are not documented at all?
That is the normal starting position and it is not a problem. Documentation is an output of the work, not a prerequisite for it, and starting from nothing is often cleaner than starting from a document that stopped being true two years ago.
Do we have to change our software?
Usually not, and we will push back on buying new tools before the underlying process works. Automating a broken workflow makes it break faster and more expensively.
